Form of Supplementary Financial Services Guide
(1) At the beginning of a Supplementary Financial Services Guide there must be: (a) a statement that it is a Supplementary Financial Services Guide; and (b) an identification of the Financial Services Guide that it supplements; and (c) a statement that it is to be read together with that Financial Services Guide and any other specified Supplementary Financial Services Guides. (2) The Supplementary Financial Services Guide must be dated. The date must be the date on which the Supplementary Financial Services Guide was prepared or its preparation was completed. (3) If the Supplementary Financial Services Guide will or may be distributed by an authorised representative of a financial services licensee, it must contain a statement to the effect that its distribution by the authorised representative has been authorised by the licensee.
