Form of Supplementary Product Disclosure Statement
At the beginning of a Supplementary Product Disclosure Statement there must be: (a) a statement that it is a Supplementary Product Disclosure Statement; and (b) an identification of the Product Disclosure Statement that it supplements; and (c) a statement that it is to be read together with that Product Disclosure Statement and any other specified Supplementary Disclosure Statements.
